Job Summary
We are seeking a compassionate and dedicated Geriatric Nurse to join our healthcare team in providing specialized care for elderly patients. The Geriatric Nurse will play a vital role in supporting elderly patients with age-related health conditions, chronic illnesses, and activities of daily living, promoting quality of life, dignity, and independence. This role requires expertise in elderly care, patience, and the ability to coordinate with multidisciplinary teams to provide holistic support to our patients.
Key Responsibilities
Patient Assessment & Care Planning
Conduct thorough health assessments and develop individualized care plans for elderly patients.
Monitor patient conditions, including vital signs, physical and mental health, and report changes to the healthcare team.
Provide patient-centered care that addresses the unique health and emotional needs of older adults.
Chronic Condition Management
Assist patients in managing chronic diseases such as diabetes, hypertension, arthritis, dementia, and mobility issues.
Administer medications, monitor effects, and educate patients and families on medication adherence and side effects.
Provide wound care, pain management, and physical assistance with daily activities as needed.
Family & Patient Education
Educate patients and their families on aging health issues, treatment plans, nutrition, and preventive health measures.
Offer guidance on managing age-related conditions and coping with lifestyle changes.
Support families in understanding end-of-life care options and advance care planning.
Emotional & Psychosocial Support
Build trusting relationships with patients, providing emotional support and maintaining dignity.
Address psychosocial aspects of elderly care, such as loneliness, depression, and anxiety.
Facilitate a comforting environment that promotes social interaction and a sense of belonging.
Coordination with Multidisciplinary Teams
Collaborate with doctors, therapists, social workers, and other healthcare professionals to deliver comprehensive care.
Participate in case discussions and contribute to care planning and discharge planning for elderly patients.
Refer patients to community resources, rehabilitation services, or support groups as needed.
Safety & Mobility Support
Assist patients with mobility and ambulation, using safe techniques to prevent falls and injuries.
Ensure the safe use of assistive devices and educate patients and families on their proper use.
Monitor and maintain patient safety, especially for those with cognitive impairments or limited mobility.
Documentation & Compliance
Maintain accurate, detailed patient records and document assessments, treatments, and observations.
Adhere to hospital protocols, infection control practices, and regulatory requirements for elderly care.
Respect patient confidentiality and uphold ethical standards in patient care.
Qualifications & Requirements
Diploma or Degree in Nursing (Bachelor of Science in Nursing preferred).
Current registration with the Nursing Council of Kenya.
Minimum of 2 years of experience in elderly or geriatric nursing preferred.
Experience with chronic disease management, palliative care, and mobility support.
Certification in Geriatric Nursing, Basic Life Support (BLS), and Advanced Cardiovascular Life Support (ACLS) is an advantage.
Strong assessment, problem-solving, and critical thinking skills.
Excellent communication skills and patience in interacting with elderly patients and their families.
Ability to work independently and as part of a multidisciplinary team.
